Androgen or testosterone is an important masculine hormone. It helps construct the male appearance. Hyperandrogenemia is majorly described in of females with hirsutism, insulin resistance and polycystic ovarian syndrome. However, hyperandrogenemia in males is not frequently mentioned. In this short article, the authors report a case of hyperandrogenemia due to ingestion of Butea superba, a herb found in South East Asia. This is an interesting case of hyperandrogenemia induced by an external source. The effect of phytoandrogens is also discussed.

The patient was a Thai single male, aged 35 years, without any underlying disease (his basic annual laboratory checkup showed normal results). On presentation, the chief complaint of this patient was a feeling of increased sexual drive. He gave the history of no use of narcotic and regular intake of vitamin and nutritional supplementation. Physical examination revealed no significant abnormality. Laboratory investigations were performed which showed increased dihydrotestosterone (1512 pg/mL, reference value 250–990 pg/mL). The results of other sexual hormone related investigations in this case included dehydroepiandrosterone sulfate 328 μg/dL, free testosterone 1.7% and sex hormone binding globulin 43.24 nmol/L [no data on follicle stimulating hormone (FSH) and luteinizing hormone (LH) levels]. The diagnosis of hyperandrogenemia in this case was therefore arrived at. Further investigation to find the source of androgen in this case was performed. With complete history taking, the clinical nutritionist could define an important problematic food component, B. superba, a local herb. The patient gave additional information that he had just taken this locally made capsule of this herb for a few weeks because he was suffering from hair loss. This patient was advised to stop ingestion of this herb, and follow-up after 1 week revealed that the patient had no feeling of increased sexual drive and dihydrotestosterone had decreased to normal level.

In general, androgen plays an important role in the sexual drive of males. Decreased androgen level is strongly related to reduced sexual activity and decreased sexual drive. This is a common problem in old males and in cases with erectile dysfunction. In males with the problem of sexual desire, androgen investigation is a useful test. In this case report, the patient also complained of increased sexual desire, which hmight have been due to exogenous hyperandrogenemeia.. Similar to hyperestrogenemia caused due to phytoestrogens, some herbal regimens might contain phytoandrogens that can lead to hyperandrogenemia. In this case, B. superba is the problematic external source of excessive androgen. This plant is considered to be a male potency herb. In animal models receiving this herb, stimulation of sexual organ has been reported. In case of human beings, there is only one previous trial using this herb for treating erectile dysfunction. The effect of this herb is comparable to that of sildenafil. However, there has never been any report on this herb in healthy males. This is the first case report on hyperandrogenemia due to ingestion of B. superba. With the widespread usage of local herbs presently, the effects of herbs need to be considered. Indeed, a previous report also mentioned genotoxicity due to large dosage ingestion of B. superba.

In 2005, the world was introduced to reclusive billionaire Jeffrey Epstein, friend to princes and an American president, a power broker with the darkest of secrets: He was also a pedophile, accused of recruiting dozens of underage girls into a sex-slave network, buying their silence and moving along, although he has been convicted of only one count of soliciting prostitution from a minor. Visitors to his private Caribbean island, known as “Orgy Island,” have included Bill Clinton, Prince Andrew and Stephen Hawking.

According to a 2011 court filing by alleged Epstein victim Virginia Roberts Giuffre, she saw Clinton and Prince Andrew on the island but never saw the former president do anything improper. Giuffre has accused Prince Andrew of having sex with her when she was a minor, a charge Buckingham Palace denies.

“Epstein lives less than one mile away from me in Palm Beach,” author James Patterson tells The Post. In the 11 years since Epstein was investigated and charged by the Palm Beach police department, ultimately copping a plea and serving 13 months on one charge of soliciting prostitution from a 14-year-old girl, Patterson has remained obsessed with the case.

“He’s a fascinating character to read about,” Patterson says. “What is he thinking? Who is he?”

Patterson’s new book, “Filthy Rich: A Powerful Billionaire, the Sex Scandal That Undid Him, and All the Justice That Money Can Buy,” is an attempt to answer such questions. Co-authored with John Connolly and Tim Malloy, the book contains detailed police interviews with girls who alleged sexual abuse by Epstein and others in his circle. Giuffre alleged that Epstein’s ex-girlfriend Ghislaine Maxwell, daughter of the late media tycoon Robert Maxwell, abused her. Ghislaine Maxwell has denied allegations of enabling abuse.

Epstein has spent the bulk of his adult life cultivating relationships with the world’s most powerful men. Flight logs show that from 2001 to 2003, Bill Clinton flew on Epstein’s private plane, dubbed “The Lolita Express” by the press, 26 times. After Epstein’s arrest in July 2006, federal tax records show Epstein donated $25,000 to the Clinton Foundation that year.

Epstein was also a regular visitor to Donald Trump’s Mar-a-Lago, and the two were friends. According to the Daily Mail, Trump was a frequent dinner guest at Epstein’s home, which was often full of barely dressed models. In 2003, New York magazine reported that Trump also attended a dinner party at Epstein’s honoring Bill Clinton.

Last year, The Guardian reported that Epstein’s “little black book” contained contact numbers for A-listers including Tony Blair, Naomi Campbell, Dustin Hoffman, Michael Bloomberg and Richard Branson.

In a 2006 court filing, Palm Beach police noted that a search of Epstein’s home uncovered two hidden cameras. The Mirror reported that in 2015, a 6-year-old civil lawsuit filed by “Jane Doe No. 3,” believed to be the now-married Giuffre, alleged that Epstein wired his mansion with hidden cameras, secretly recording orgies involving his prominent friends and underage girls. The ultimate purpose: blackmail, according to court papers.

“Jane Doe No. 3” also alleged that she had been forced to have sex with “numerous prominent American politicians, powerful business executives, a well-known prime minister, and other world leaders.”

“The reader has to ask: Was justice done here or not?”

Epstein, now 63, has always been something of an international man of mystery. Born in Brooklyn, he had a middle-class upbringing: His father worked for the Parks Department, and his parents stressed hard work and education.

Epstein was brilliant, skipping two grades and graduating Lafayette High School in 1969. He attended Cooper Union but dropped out in 1971 and by 1973 was teaching calculus and physics at Dalton, where he tutored the son of a Bear Stearns exec. Soon, Epstein applied his facility with numbers on Wall Street but left Bear Stearns under a cloud in 1981. He formed his own business, J. Epstein & Co.

The bar for entry at the new firm was high. According to a 2002 profile in New York magazine, Epstein only took on clients who turned over $1 billion, at minimum, for him to manage. Clients also had to pay a flat fee and sign power of attorney over to Epstein, allowing him to do whatever he saw fit with their money.

Still, no one knew exactly what Epstein did, or how he was able to amass a personal billion-dollar-plus fortune. In addition to a block-long, nine-story mansion on Manhattan’s Upper East Side, Epstein owns the $6.8 million mansion in Palm Beach, an $18 million property in New Mexico, the 70-acre private Caribbean island, a helicopter, a Gulfstream IV and a Boeing 727.

“My belief is that Jeff maintains some sort of money-management firm, though you won’t get a straight answer from him,” one high-level investor told New York magazine. “He once told me he had 300 people working for him, and I’ve also heard that he manages Rockefeller money. But one never knows. It’s like looking at the Wizard of Oz — there may be less there than meets the eye.”

“He’s very enigmatic,” Rosa Monckton told Vanity Fair in 2003. Monckton was the former British CEO of Tiffany & Co. and confidante to the late Princess Diana. She was also a close friend of Epstein’s since the 1980s. “He never reveals his hand .?.?. He’s a classic iceberg. What you see is not what you get.”

Both profiles intimated that Epstein had a predilection for young women but never went further. In the New York magazine piece, Trump said Epstein’s self-professed image as a loner, an egghead and a teetotaler was not wholly accurate.

“I’ve known Jeff for 15 years,” Trump said. “Terrific guy. He’s a lot of fun to be with. It is even said that he likes beautiful women as much as I do, and many of them are on the younger side. No doubt about it — Jeffrey enjoys his social life.”

Three years after that profile ran, Palm Beach Police Officer Michele Pagan got a disturbing message. A woman reported that her 14-year-old stepdaughter confided to a friend that she’d had sex with an older man for money. The man’s name was Jeff, and he lived in a mansion on a cul-de-sac.

Pagan persuaded the woman to bring her stepdaughter down to be interviewed. In his book, Patterson calls the girl Mary. And Mary, like so many of the other girls who eventually talked, came from the little-known working-class areas surrounding Palm Beach.

A friend of a friend, Mary said, told her she could make hundreds of dollars in one hour, just for massaging some middle-aged guy’s feet. Lots of other girls had been doing it, some three times a week.

Mary claimed she had been driven to the mansion on El Brillo Way, where a female staffer escorted her up a pink-carpeted staircase, then into a room with a massage table, an armoire topped with sex toys and a photo of a little girl pulling her underwear off.

Epstein entered the room, wearing only a towel, Mary said.

“He took off the towel,” Mary told Pagan. “He was a really built guy. But his wee-wee was very tiny.”

Mary said Epstein got on the table and barked orders at her. She told police she was alone in the room with him, terrified.

Pagan wrote the following in her incident report:

“She removed her pants, leaving her thong panties on. She straddled his back, whereby her exposed buttocks were touching Epstein’s exposed buttocks. Epstein then turned to his side and started to rub his penis in an up-and-down motion. Epstein pulled out a purple vibrator and began to massage Mary’s vaginal area.”

Palm Beach assigned six more detectives to the investigation. They conducted a “trash pull” of Epstein’s garbage, sifting through paper with phone numbers, used condoms, toothbrushes, worn underwear. In one pull, police found a piece of paper with Mary’s phone number on it, along with the number of the person who recruited her.

On Sept. 11, 2005, detectives got another break. Alison, as she’s called in the book, told Detective Joe Recarey that she had been going to Epstein’s house since she was 16. Alison had been working at the Wellington Green Mall, saving up for a trip to Maine, when a friend told her, “You can get a plane ticket in two hours .?.?. We can go give this guy a massage and he’ll pay $200,” according to her statement to the police.

Alison told Recarey that she visited Epstein hundreds of times. She said he had bought her a new 2005 Dodge Neon, plane tickets, and gave her spending money. Alison said he even asked her to emancipate from her parents so she could live with him full-time as his “sex slave.”

She said Epstein slowly escalated his sexual requests, and despite Alison’s insistence that they never have intercourse, alleged, “This one time .?.?. he bent me over the table and put himself in me. Without my permission.”

Alison then asked if what Epstein had done to her was rape and spoke of her abject fear of him.

An abridged version of her witness statement, as recounted in the book:

Alison: Before I say anything else .?.?. um, is there a possibility that I’m gonna have to go to court or anything?
Recarey: I mean, what he did to you is a crime. I’m not gonna lie to you.
Alison: Would you consider it rape, what he did?
Recarey: If he put himself inside you without permission .?.?. That, that is a crime. That is a crime.
Alison: I don’t want my family to find out about this .?.?. ’Cause Jeffrey’s gonna get me. You guys realize that, right? .?.?. I’m not safe now. I’m not safe.
Recarey: Why do you say you’re not safe? Has he said he’s hurt people before?
Alison: Well, I’ve heard him make threats to people on the telephone, yeah. Of course.
Recarey: You’re gonna die? You’re gonna break your legs? Or?—
Alison: All of the above!

Alison also told Recarey that Epstein got so violent with her that he ripped out her hair and threw her around. “I mean,” she said, “there’s been nights that I walked out of there barely able to walk, um, from him being so rough.”

Two months later, Recarey interviewed Epstein’s former house manager of 11 years, documented in his probable-cause affidavit as Mr. Alessi. “Alessi stated Epstein receives three massages a day .?.?. towards the end of his employment, the masseuses .?.?. appeared to be 16 or 17 years of age at the most . . . [Alessi] would have to wash off a massager/vibrator and a long rubber penis, which were in the sink after the massage.”

Another house manager, Alfredo Rodriguez, told Recarey that very young girls were giving Epstein massages at least twice a day, and in one instance, Epstein had Rodriguez deliver one dozen roses to Mary, at her high school.

In May 2006, the Palm Beach Police Department filed a probable-cause affidavit, asking prosecutors to charge Epstein with four counts of unlawful sexual activity with a minor — a second-degree felony — and one count of lewd and lascivious molestation of a 14-year-old minor, also a second-degree felony.

Palm Beach prosecutors said the evidence was weak, and after presenting the case to a grand jury, Epstein was charged with only one count of felony solicitation of prostitution. In 2008, he pleaded guilty and nominally served 13 months of an 18-month sentence in a county jail: Epstein spent one day a week there, the other six out on “work release.”

Today, Jeffrey Epstein is a free man, albeit one who routinely settles civil lawsuits against him, brought by young women, out of court. As of 2015, Epstein had settled multiple such cases.

Giuffre has sued Ghislaine Maxwell in Manhattan federal court, charging defamation — saying Maxwell stated Giuffre lied about Maxwell’s recruitment of her and other underage girls. Epstein has been called upon to testify in court this month, on Oct. 20.

The true number of Epstein’s victims may never be known.

He will be a registered sex offender for the rest of his life, not that it fazes him.

“I’m not a sexual predator, I’m an ‘offender,’?” Epstein told The Post in 2011. “It’s the difference between a murderer and a person who steals a bagel.”

Kim Jeong-oh left her home in North Korea to start a new life across the Yalu river. Instead of finding a job, however, the 35-year-old was sold as a wife for £390. Along with countless others, she fled a devastating famine in her native town of Kimchaek on the advice of a guide who offered to arrange her passage to China.

But when Ms Kim arrived in the border town of Hyesan, the North Korean guide arranged to sell her to a wife-trader. "He promised to find me a factory job in China where I could earn 2,000RMB (£160) per month," she said. "I had no idea he was planning to sell me."

She and three other women waded across the shallow river and were met by a Chinese broker who paid 300RMB for each of them. They spent the next four days in a car parked in the mountains while their "owner" drove from village to village looking for buyers.

"I was sold to the first bidder for 5,000RMB," Ms Kim, whose name has been changed to protect her identity, said. "I don't know what happened to the other girls."

There is no shortage of men in need of a wife in the rustbelt of northeast China, where migrant workers labour far from home in thousands of tiny coal mines. Villages are emptying of young people who would rather seek their fortunes in the cities.

"There aren't enough women here," said a middle-aged local. "All the pretty girls leave to become prostitutes. For many men, a Korean wife is very desirable."

The practice of wife-buying is illegal but commonplace. Towns and villages from the border area to the city of Shenyang in Liaoning province are filled with tales of wife sales.

Not all go unwittingly into the marriage market. Food and shelter are considerable incentives, but the risks are immense. As illegal immigrants, the women can be arrested at any time and sent back to North Korea.

Ms Kim was picked up a year after getting married and giving birth to a daughter. Her new family pleaded for her release, arguing that the baby needed her mother because she was still breastfeeding. Ms Kim says they paid a 10,000RMB bribe for her freedom. Three years later she is well established and has a residence permit.

The trade in women is said to have fallen in the past two years as the food situation improves in North Korea and Chinese police crack down. But locals are adamant that the business continues.

Chandler, Arizona: How Chickens Lost Their Penises (And Ducks Kept Theirs)

If youíve never seen a duck penis before, have a look at the infamous video above. That long corkscrew belongs to a Muscovy duck, and itís typical of the group. Some ducks have helical penises that are longer than their entire bodies. But forget the helical shape, the size, and the surprisingly explosive extensionóthe weirdest thing about a duckís penis is that it has one.

Most birds donít. There are almost 10,000 species of birds and only around 3 percent of them have a penis. These include ducks, geese and swans, and large flightless birds like ostriches and emus. But eagles, flamingos, penguins and albatrosses have completely lost their penises. So have wrens, gulls, cranes, owls, pigeons, hummingbirds and woodpeckers. Chickens still have penises, but barelyótheyíre tiny nubs that are no good for penetrating anything.

In all of these species, males still fertilise a femaleís eggs by sending sperm into her body, but without any penetration. Instead, males and females just mush their genital openings together and he transfers sperm into her in a manoeuvre called the ìcloacal kissî. Two dunnocks demonstrate the move in the video below. If you blink at 00:36, you will miss it.

ìThere are lots of examples of animal groups that evolved penises, but I can think of only a bare handful that subsequently lost them,î says Diane Kelly from the University of Massachusetts in Amherst. ìOrnithologists have tied themselves in knots trying to explain why an organ that gives males an obvious selective advantage in so many different animal species disappeared in most birds. But itís hard to address a question on why something happens when you donít know much about how it happens.î

Thatís where Martin Cohn came in. He wanted to know the how. His team at the University of Florida studies how limbs and genitals develop across the animal kingdom, from the loss of legs in pythons to genital deformities in humans. ìIn a lab that thinks about genital development, one takes notice when a species that reproduces by internal fertilization lacks a penis,î says graduate student Ana Herrera.

Waves of death

By comparing the embryos of a Pekin duck and a domestic chicken, Herrera and other team members showed that their genitals start developing in the same way. A couple of small swellings fuse together into a stub called the genital tubercle, which gradually gets bigger over the first week or so. (The same process produces a mammalís penis.)

In ducks, the genital tubercle keeps on growing into a long coiled penis, but in the chicken, it stops around day 9, while itís still small. Why? Cohn expected to find that chickens are missing some critical molecule. Instead, his team found that all the right penis-growing genes are switched on in the chickenís tubercle, and its cells are certainly capable of growing.

It never develops a full-blown penis because, at a certain point, its cells start committing mass suicide. This type of ëprogrammed cell deathí occurs throughout the living world and helps to carve away unwanted body partsófor example, our hands have fingers because the cells between them die when weíre embryos. For the chicken, it means no penis. ìIt was surprising to learn that outgrowth fails not due to absence of a critical growth factor, but due to presence of a cell death factor,î says Cohn.

His team confirmed this pattern in other species, including an alligator (crocodilians are the closest living relatives of birds). In the greylag goose, emu and alligator, the tubercle continues growing into a penis, with very little cell death. In the quail, a member of the same order as chickens, the tubercleís cells also experience a wave of death before the organ can get big.

This wave is driven by a protein called Bmp4, which is produced along the entire length of the chickenís tubercle, but over much less of the duckís. When Cohnís team soaked up this protein, the tubercleís cells stopped dying and carried on growing. So, itís entirely possible for a chicken to grow a penis; itís just that Bmp4 stops this from happening. Conversely, adding extra Bmp protein to a duck tubercle could stop it from growing into its full spiralling glory, forever fixing it as a chicken-esque stub.

Bmp proteins help to control the shape and size of many body parts. Theyíre behind the loss of wings in soldier ants and teeth of birds. Meanwhile, bats blocked these proteins to expand the membranes between their fingers and evolve wings.

They also affect the genitals of many animals. In ducks and geese, they create the urethra, a groove in the penis that sperm travels down (ìIf you think about it, thatís like having your urethra melt your penis,î says Kelly.) In mice, getting rid of the proteins that keep Bmp in check leads to tiny penises. Conversely, getting rid of the Bmp proteins leads to a grossly enlarged (and almost tumour-like) penis.

To lose a penis once might be regarded as misfortuneÖ

Penises have been lost several times in the evolution of birds. Cohnís team have only compared two groupsóthe penis-less galliforms (chickens, quails and pheasants) and the penis-equipped anseriforms (swans, ducks and geese). What about the oldest group of birdsóthe ratites, like ostriches or emus? All of them have penises except for the kiwis, which lost theirs. And what about the largest bird group, the neoaves, which includes the vast majority of bird species? All of them are penis-less.

Maybe, all of these groups lost their penis in different ways. To find out, Herrera is now looking at how genitals develop in the neoaves. Other teams will no doubt follow suit. ìThe study will now allow us to more deeply explore other instances of penis loss and reduction in birds, to see whether there is more than one way to lose a penis,î says Patricia Brennan from the University of Massachussetts in Amherst, the woman behind the duck penis video at the top.

And in at least one case, what was lost might have been regained. The cracidsóan group of obscure South American galliformsóhave penises unlike their chicken relatives. It might have been easy for them to re-evolve these body parts, since the galliforms still have all the genetic machinery for making a penis.

But why?

We now know how chickens lost their penises, but we donít know why a male animal that needs to put sperm inside a female would lose the organ that makes this possible. Cohnís study hints at one possibilityóit could just be a side effect of other bodily changes. Bmp4 and other related proteins are involved in the evolution of many bird body parts, including the transition from scales to feathers, the loss of teeth, and variations in beak size. Perhaps one of these transformations changed the way Bmp4 is used in the genitals and led to shrinking penises.

There are many other possible explanations. Maybe a penis-less bird finds it easier to fly, runs a smaller risk of passing on sexually-transmitted infections, or is better at avoiding predators because he mates more quickly (remember the dunnocks?).

Females might even be responsible. Male ducks often force themselves upon their females but birds without an obvious phallus canít do that. They need the femaleís cooperation in order to mate. So perhaps females started preferring males with smaller penises, so that they could exert more choice over whom fathered their chicks. (Indeed, the now-infamous story about the duckís corkscrew penis is really a story about the duckís corkscrew vagina.) Combinations of these explanations may be right, and different answers may apply to different groups.

And why study the why? Why would scientists care about how penises evolve (and why do I write about them so much)? Cohn makes a good argument. ìGenitalia are one of the fastest-evolving organs in animals,î he explains. Even in the groups with backbones, ìone sees a remarkable degree of variationî.

In mammals, sperm passes down a tube thatís fully enclosed within the penis; in birds and reptiles, it goes down an open groove. Some mammals have a bone in the penis, or a coat of spines; others donít. Snakes, lizards and kangaroos have two-pronged penises, while echidnas have four-pronged ones. Mammals inflate their penises with blood; birds use lymph; alligators have a permanently erect penis connected to a bungee cord. When Darwin spoke of ìendless forms most beautiful and most wonderfulî, itís easy to envisage him talking about penises. Or a lack of them.

Following the high-profile case of Anthony Weiner, the U.S. Attorneys' Offices have successfully prosecuted another case under the Project Safe Childhood initiative.

A former Secret Service officer has been sentenced to 20 years in prison, followed by a lifetime of supervised release, for conducting sexual conversations with a minor and attempting the exchange of explicit images.

Lee Robert Moore, 38, of Church Hill, Md., pleaded guilty March 1, 2017, after Delaware State Police with the Delaware Child Predator Task Force had sexual chats online with Moore, at times when he was a work, and were requested to send him explicit photos while posing as a 14-year-old girl.
