Many women complain that they have a bad smell coming from the vagina, even after washing frequently. This can be very distressing, particularly if it is noticed and commented on by someone else.
Genital odour is due to the combination of vaginal secretions, eccrine and apocrine sweat and external sources (urine, faeces, topical applications).
What symptoms should lead to concern?
A bad smell could be due to genital infection or disease. Clues include:
excessive vaginal discharge itching (pruritus vulvae) pain and soreness.
What conditions cause vaginal malodour?
Sometimes the apparently bad vaginal smell is actually normal, as vaginal secretions in every adult woman have a rather musty odour. The smell can vary throughout the menstrual cycle. There is also a wide variation in what is considered acceptable.
Bad smell is however often associated with infectious or non-infectious causes of vaginitis or less often, vulval disease.
Malodorous vaginal infections include:
Bacterial vaginosis (the most common reason for genital malodour, a fishy smell) Trichomoniasis (this is foul-smelling in only about 20% of infected women) Vulval ulceration of any cause, particularly if due to donovanosis or chancroid Vaginal discharge associated with pelvic inflammatory disease Forgotten foreign bodies such as tampons, diaphragms or sponges Fistulas or passageways linking the vagina with the rectum or bladder following childbirth, injury or surgery Hidradenitis suppurativa. Although candidal vulvovaginitis (thrush) is very common, it causes a yeasty smell, which is not considered particularly unpleasant by most women.
Noninfectious causes of vaginal malodour include:
Excessive perspiration ( hyperhidrosis leading to bromhidrosis) especially associated with obesity Chronic constipation and bloating or dietary factors leading to release of smelly rectal gases Urinary incontinence, releasing ammonia Faecal incontinence Poor hygiene, often in women who are elderly or mentally unwell Vulval cancer, when it is due to necrosis (death of tissue) Discharge or necrosis of other genital cancers Trimethylaminuria (fish-odour syndrome) Olfactory hallucinations, e.g. associated with temporal lobe epilepsy Psychiatric conditions. What tests should be done?
Women complaining of genital malodour should undergo careful external and internal examination after a careful history has been taken. Tests may include pH, vaginal and/or vulval swabs for microbiology and sometimes skin biopsy.
Treatment depends on the underlying cause. Antibiotics should be prescribed for confirmed infection.
General measures should include:
Avoid wearing tight or occlusive underwear Change underwear frequently Bathe gently using non-soap cleanser once or twice daily Attempt to lose weight, if relevant If incontinent of urine, copper acetate impregnated incontinence pads may help to reduce the smell. The hazards of self-treatment
Excessive washing, antiseptics, deodorants and douching (rinsing out the vagina) may irritate the vagina and vulva, potentially resulting in increased irritation and discharge from vulvitis, chemically-induced vaginitis or secondary infection. Don’t do it!

Muncie, Indiana: Man killed himself after Stinson Hunter 'paedophile trap'

Michael Parkes, 45, was confronted and filmed in May last year by Stinson Hunter, who had arranged to meet him after posing as a child online.
Mr Parkes was later arrested by police but had not been charged.
Northamptonshire Coroner Anne Pember recorded a verdict of suicide, with the cause of death as hanging.

Mr Parkes was arrested by Northamptonshire Police on 29 May last year on suspicion of meeting someone he thought to be a 12-year-old girl for sex.

In a statement after the inquest, the force said it would have sought a charging decision based on the evidence supporting the allegation.
A force spokesman said: "It is clear that Parkes drove to Coventry with the intention of engaging in sexual activity with a child."
Stinson Hunter, who changed his name from Keiren Parsons, has caused controversy by posting videos of his meetings online and has been criticised by police for his methods.
At the inquest at Northampton General Hospital, the coroner said Mr Parkes, a warehouse manager and father of one, was "feeling low and depressed" at the time of his death on 2 June last year.
The inquest heard he was found dead in his car with a ligature around his neck.
Mr Parkes, who lived with his mother in Hemans Road, Daventry, was removed from the car. A passing doctor led efforts to resuscitate him but he died at the scene.
Dr Alexander Wennekes, a GP, said he had prescribed anti-depressants to Mr Parkes for bouts of work-related stress and depression.
Dr Wennekes said that earlier in 2013, Mr Parkes complained of feeling depressed after splitting from his partner of three years while she was pregnant with their son.
He said Mr Parkes, who also had a wife from whom he was separated, contacted the surgery again on 30 May, saying he was suffering depression.

"When I saw him he said he was going through a bad patch, had financial difficulties, no access to his son and had been arrested by the police on what he said were false allegations - however, he did not explain further," said Dr Wennekes.
Mr Parkes' brother Richard said the arrest "had a significant effect on his state of mind and the actions he had taken".
Following the inquest, he said: "The police have not provided me with any evidence that supports these allegations."
Paying tribute to Mr Parkes, he said: "Michael's family are deeply shocked by his death.
"He was a loving father, son and brother and a caring person who put his family and others first."
After the inquest, Mr Hunter, from Warwickshire, said he did not feel responsible for Mr Parkes' death.

"I feel for his family and it's a shame for them, but ultimately he made his own choices," he said.
"I have done the right thing. He had been arrested and bailed."
Northamptonshire Police offered its condolences to Mr Parkes' family.
"We have a duty to investigate and respond to child abuse allegations," a spokesman said.
"A decision to arrest Parkes was made when it became clear from the material supplied to us that he believed the age of the person he was in communication with was 12 years old.
"In the light of Parkes' subsequent death, a thorough review was carried out and it is clear that it would have passed the threshold test and that Northamptonshire Police would have sought a CPS charging decision."
The spokesman added: "We do not condone or support the actions of Stinson Hunter."
Separately, Warwickshire Police and West Mercia Police issued a joint statement stating they did not condone his methods and adding that his actions "could have several serious consequences", including compromising continuing investigations.

The results showed that all the root extracts except the water extract of E. longifolia produced significant cytotoxic effect on these cell lines. However, no significant cytotoxic effect was detected on MDBK (kidney) normal cell line. 9-methoxycanthin-6-one, an alkaloid, was detected in each extract with different intensities by reversed-phase high performance liquid chromatography. You can have a six-pack sculpted, dimples created, a designer vagina and almost any part of your face tweaked, lifted and tightened.
But there’s a new plastic surgery procedure that is on the rise — and it’s altogether more bizarre.
The number of men inquiring about scrotox — yes, that’s having Botox injected into your scrotum — has doubled in the past year, according to experts.
The operation, which can cost up to $3,600, helps ease sweating, lessens the appearance of wrinkles and makes the scrotum appear larger by helping the muscles relax, the Metro reports.
It is already a staple on the menu at clinics across the US, and with the UK around three years behind the plastic surgery times, experts say they expect to see a surge in interest in the coming years.
Mark Norfolk, clinical director at Transform, a national clinic, said that even though they do not offer Scrotox, they have seen a huge rise in the number of patients asking about the procedure.
He told The Sun Online: “Over the past year, requests for scrotum Botox have doubled at Transform, showing the huge demand and interest for this procedure.”
He said the procedure is not offered at Transform “due to the possible risks and complications associated with treating this part of the body.”
The procedure involves Botox being injected into a man’s scrotum.
While the drug is commonly used slightly higher up the anatomy, to rid foreheads and faces of unsightly wrinkles, Norfolk warns it does not have the same effect below the waistline.
“In terms of results, injecting Botox into the scrotum may help with any sweating issues, but won’t have much of an effect on wrinkles,” he explained.
“There is lots of loose skin on this part of the body, that an injectible treatment just can’t shift.”
The only way to get rid of excessive wrinkles and loose skin on a man’s scrotum is to have surgery to remove the excess skin, Norfolk said.
He added: “If anyone is interested in having this treatment, I can’t stress enough how important it is to do a thorough research — not only into the practitioner but also around the product they’ll be using.”
“Also, patients should manage their expectations in terms of results, it could prove very costly and nerve-wracking to go through, for very little in return.”
Writing for the Cosmetic Surgery Times, Dr. Jason Emer, a surgeon based in Beverley Hills, California, said that like the advent of the designer vagina, it is likely Scrotox will go from being a “hush-hush” op to one that is trending.
He said earlier this year: “As the vaginal rejuvenation market is skyrocketing, men are seeking their own type of rejuvenation.”
“Who wouldn’t want to be a little bit longer, thicker, or have more sensitivity and a better sex life?”
“These men are also becoming interested in the cosmetic appearance of the actual penis and scrotum itself.”
Emer said he has seen a rise in the number of men seeking advice on the op, and adds that he expects that number to continue to grow.

A terminally ill pensioner who had made arrangements with the Dignitas euthanasia clinic in Switzerland took his own life in his home, fearing he would be too ill to travel.
Lawrence Klein was a keen marathon runner and hillwalker when he was younger, but in 2014 his balance became increasingly unstable.
After being seen by audiology specialists, the 74-year-old from Reading was referred to neurology consultants at John Radcliffe Hospital in Oxford. They conducted MRI scans, but they were inconclusive.
As his health rapidly deteriorated, Mr Klein was told in early 2016 that the possible cause of his problems was corticobasal deterioration (CBD) – a rare and incurable illness which becomes progressively worse.
His wife Martha agreed to accompany him to a provisional appointment at Dignitas, but just a few days before they were due to travel to Sexual Switzerland he hanged himself while his wife was out shopping, an inquest heard.
Peter Bedford, senior coroner for Berkshire, told Mrs Klein: ‘You described how, for the three years before his death, Mr Klein’s balance became poor, he found driving exhausting, but what concerned him most was his balance.
‘These changes in his health were significant. He had previously been very active and ran marathons and half marathons, and enjoyed hill walking.’
By the time of his diagnosis, Mr Bedford continued, ‘Your husband could no longer walk a mile without becoming tired, he could no longer read for more than a few minutes’, adding that Mr Klein had contacted Dignitas because ‘he did not want to become totally dependent on others for his care’.
In a statement read out to Reading Coroner’s Court, Mrs Klein said:
‘My husband was becoming increasingly disabled. He was worried he would not be physically able to go to Switzerland.’

On the morning of June 10, Mr Klein had agreed that Martha should phone his sisters and let them know about his plan to end his life by euthanasia, which had until then been kept between the two of them.
Mrs Klein went out to go shopping that afternoon – and when she returned, she found her husband dead.
He had not left a handwritten note, but he had sent ‘goodbye’ emails to family and friends.
‘He sent the emails as he could no longer write by hand,’ the coroner said.

Mucuna pruriens cotyledon powder signiﬁcantly increased the brain mitochondrial complex-I activity but did not affect the total monoamine oxidase activity (in vitro). Unlike synthetic levodopa treatment, Mucuna pruriens cotyledon powder treatment signiﬁcantly restored the endogenous levodopa, dopamine, norepinephrine and serotonin content in the substantia nigra. Nicotine adenine dinucleotide (NADH) and coenzyme Q-10, that are shown to have a therapeutic beneﬁt in Parkinson's disease, were present in the Mucuna pruriens cotyledon powder. Earlier studies showed that Mucuna pruriens treatment controls the symptoms of Parkinson's disease. This additional ﬁnding of a neurorestorative beneﬁt by Mucuna pruriens cotyledon powder on the degenerating dopaminergic neurons in the substantia nigra may be due to increased complex-I activity and the presence of NADH and coenzyme Q-10.

A mushroom of the dictyophora family, and synonymous with Hawaii, has been found to help women orgasm.
Known as phallus indusiatus, the mushroom cunningly resembles a man’s phallus and is dressed up in a fishnet like covering.
A study, published in the International Journal of Medicinal Mushrooms in 2001, revealed the smell from the mushroom triggered spontaneous orgasms.
The research was conducted by John Holliday of Next Laboratories in Kula, Hawaii and Noah Soule of Aloha Medicinals.
The duo tested the mushroom’s aphrodisiac effect in an experiment involving 16 women and 20 men.
The volunteers were asked to smell the mushroom, which is said to have a ‘fetid odour’.
Six women had orgasms, while the other 10, who received smaller doses, experienced an increased heart rate.
The study explained: “There are significant sexual arousal characteristics present in the fetid odour of this unique mushroom.
“These results suggest that the hormone like compounds present in the volatile portion of the spore mass may have some similarity to human neurotransmitters during sexual encounters.”
Phallus indusiatus is also found in southern Asia, Africa and Australia, where it grows in woodlands and gardens in rich soil and well-rotted woody material.
Many people equate good sex with the type of orgasm they, and their partner, have.
